You will submit the following supporting materials through this site:

Personal Statement Form
Faculty Mentor Request Form

Document Submission Guidelines:

You may only submit word documents or PDF files.
Filenames MUST have your last name and first name initial in them (e.g. SmithJPersonalStatement.pdf or GarciaEFacultyMentorRequestForm.pdf)
File names must NOT contain: spaces, hyphens, slashes, commas or other non-alphanumeric characters

Please carefully review the application process at http://sciences.ugresearch.ucla.edu/programs-and-scholarships/amgen-scholars/application/ prior to uploading your documents. (The Personal Statement Form and Faculty Mentor Request Form can be downloaded there, as well as a full application checklist.)

All application materials must be received by Tuesday, February 1, 2022.
